NATIONAL VOTER'S SERVICES PORTAL  રાષ્ટ્રીય મતદાતા સેવા પોર્ટલની સાઈટ ખુલશે.
આ સાઈટ ખુલે તેમાં...   प्रारुप 6/Form 6  નું ફોર્મ ખુલશે.
આ  ૬   નંબરના ફોર્મમાં આપેલ તમામ વિગત આ મુજબ ભરવી.

=>  जन्म स्थान का विवरण /Place of Birth Details

• राज्य चुनें/Select State*
• जिला चुनें/Select District*
• गांव/शहर/Village/ Town
• जन्म का स्थान/Birth Place

           पिता/माता/पति का नाम/Father's/Mother's/Husband's Name*

• (In English) (In Regional)

          पिता/माता/पति का उपनाम/Father's/Mother's/Husband's Surname

• संबंध/Relation*

II. मामूली तौर से निवास स्थान का विवरण (पूरा पता)/II. Particulars of place of present ordinary Residence (Full address)

III. निर्वाचन क्षेत्र की वर्तमान निर्वाचक नामावली में पहले से ही सम्मिलित किए गए आवेदक के परिवार के सदस्य(यों) का ब्यौरा:/III. Details of member(s) of applicant's family already included in the current electoral roll of the Constituenc

કુટુંબના સભ્યની વિગત સચોટ ભરવી.

• नाम/Name
• आवेदक के साथ संबंध/Relationship with applicant
• निर्वाचन-क्षेत्र की नामावली की भाग संख्या/Part no. of the roll of the Constituency
• उस भाग में क्रम संख्या/Serial number in that Part
• निर्वाचक की फोटो पहचान-पत्र संख्या/Elector's Photo Identity Card Number

IV. सहायक दस्तावेज अपलोड करे/IV. Upload Supporting Document

• अपना फोटो/ Your Photograph ફોટો.
• पहचान प्रमाण/ Identity Proof  ઓળખકાર્ડ.
• पता प्रमाण/ Address Proof  રહેણાંકણો પુરાવો.

ઉપરોક્ત ત્રણ આધાર અપલોડ કરવાના રહેશે.

V.घोषणा/V. Declaration

• मैं घोषणा करता हूँ कि अपने सर्वोत्तम ज्ञान और विश्वास के अनुसार:-/I hereby declare that to the best of my knowledge and belief: -

(i) मैं भारत का नागरिक हूँ ;/(i) I am a citizen of India;
(ii) मैं.(तारीख, मास, वर्ष) से उपर्युक्त भाग-II में दिए गए पते पर मामूली तौर से निवासी हूँ;/(ii) I am ordinarily resident at the address given in para II above since (year,month,date)

(iv)   इस या किसी अन्य विधान सभा निर्वाचन-क्षेत्र की निर्वाचक नामावली में मेरा नाम पहले से ही सम्मिलित नहीं किया गया है/My name has not already been included in the electoral roll for this or any other assembly constituency;
               या/Or
 मेरा नाम नीचे उल्लिखित राज्य व निर्वाचन क्षेत्र के लिए ,निर्वाचक नामावली में सम्मिलित कर लिया गया होगा/My name may have been included in the electoral roll

टिप्पणी:- कोई व्यक्ति ऐसा कथन या घोषण करता है जो मिथ्या है और जिसके मिथ्या होने का या तो उसे ज्ञान या विश्वास है या जिसके सत्य होने का उसे विश्वास नहीं है, वह लोक प्रतिनिधित्व अधिनियम, 1950, (1950 का 43) की धारा 31 के अधीन दंडनीय होगा।/Note - Any person who makes a statement or declaration which is false and which he either knows or believes to be false or does not believe to be true, is punishable under Section 31 of the Representation of the People Act, 1950 (43 of 1950).

ત્યાર પછી ફોર્મ સંપૂર્ણ ભરાઈ ગયા  બાદ    સેવ (SAVE)  કરો / સબમિટ (SUBMIT)  કરો.
